A Wellness Assessment is a “whole person” assessment and lifestyle program designed to help you gain personal insight into your state of physical, emotional, and spiritual wellness. This on-line assessment offers guidance and tools to transform what you have learned into lasting changes and a renewed sense of health and well-being.
The assessment, based on the work of wellness pioneer John W. Travis, MD, MPH, identifies your areas of strength, the specific areas in which you are most motivated to change, and guides you in creating a wellness action plan based upon your results. In addition, the Wellness Assessment provides tools and resources to help you reach your wellness goals and bring more balance into your life.

How Do I Take The Assessment And Get Wellness? a.k.a. What will you receive?
Step 1: You Take An On-Line Assessment. You discover your state of balance in 12 key areas. (See the 12 dimensions of wellness chart above for these areas.) You can take the assessment in one sitting, or log-off prior to completing the assessment, save your answers, and return to it at a later date.
Step 2: You Receive Your Scores: Your Wellness Scores indicate your level of wellness in each of the 12 areas. Your scores indicate your readiness for change. Low scores show you the areas that you are most motivated to change.
Step 3: You Create A Personal Wellness Plan: The Assessment provides a place to put in up to 10 action steps which are the basis and foundation for the changes you are about to make. Your plan will help you stay focused and on track.
Step 4: You Receive Tools To Help You Reach Your Wellness Goals: You will receive a progress tracker and a “Wellness Home Page” to support you in your wellness journey. Included is a suite of tools to help you follow your wellness plan and achieve your goals over one-year period. You can edit and update your plan as necessary throughout the year. In addition, you can retake the assessment in six months to evaluate your progress via the test comparison feature, which includes the percentage of change in the Wellness & Satisfaction Scores for each of the 12 areas.
What Else Do You Receive?
1. Email Reminders: You receive regular permission-based email reminders containing your current wellness plan that will motivate you and keep you focused on your goals.
2. A Great Resource Section: Each of the 12 areas has a corresponding Resource Center, so you can have access to resources that will support you in carrying out your action steps, meeting your wellness goals, and creating lasting change.

The cost includes your assessment, a 60-minute call to go over your results, and full access to your assessment, results, and resources for a full year.
What Will Happen Next?
- Once you purchase your assessment, you will receive, via e-mail, a user name and password to take your test and gain access to your resources.
